Contract: Permanent part time up to 32 hours per fortnight available - less will be considered.

About the role
This is a Health Care Assistant role that focuses on supporting the Transitional Home Support Services by supporting people who have been recently discharged from hospital and need some support to attend to household duties and activities of daily living.
The service is provided Monday to Friday and we are flexible with start times envisaging 3-4 hours per day to make up to 16 hrs per week.
You will be providing Home Help and assisting with personal hygiene and other activities of daily living for people who have recently been in Hospital.
About the Team/Service/ Location
Community Health Service team is made up of District Nurses, Specialist Nurses, Admin and Health Care Assistants and provides a variety of services with a focus on quality health care working alongside patients and their whanau to achieve optimal health outcomes.
About you
We are keen to hear from Health Care Assistants with a proven track record of working in the community with a restorative focus who is prepared to work the hours available. Career-force level 3 is an advantage although we will support the right candidate to complete this.
You will required a NZ Driver - s License for this role.
