About the role

Reporting to the Quality and Practice Manager, you will be responsible for supporting the development and transformation of organisational and front-line staff practice to ensure continual improvement in the health outcomes of people supported by Community Connections.

You will achieve this by ensuring our health practices are consistent with statutory requirements and contractual compliance. This includes identifying and reducing clinical risk, as well as leading or participating in the design, implementation, and management of clinical projects.

You will identify health risks and complete individual health reviews to inform ongoing care and referrals to relevant external health providers in order to achieve long-term health goals for the people we support. Additionally, you will provide clinical planning, support, and advice in line with the guidance of clinical health providers and develop clinical protocols.

We are looking for someone with a strong commitment to respecting the rights of and supporting those with disabilities. You will be a relationship builder committed to developing, coaching, and supporting staff to ensure that their skills and knowledge base continue to grow.

You will have a strong alignment with our purpose, vision, values, and Mana Wairoa.

Essential:
  • Registered Nurse with current Annual Practising Certificate (Registered Nurse - preferably with experience working with Intellectual Disability)
  • Minimum two years post-registration experience
  • High level of integrity and professionalism
  • Ability to maintain confidentiality and discretion
  • Strong verbal and written communication skills
  • Willingness to travel, which will be required as part of this role
Desirable
  • Knowledge of the health system and of the role and function in relation to community services and facilities and wider clinical specialties
  • Previous experience in the disability sector
  • Familiarity with the legislation governing the health and disability sector, and the care of people with disabilities
  • Knowledge of the requirements for maintaining the safety and well-being of people with disabilities
  • Experience in developing collaborative and individualised support solutions to ensure the best possible outcomes for the people we support
  • Policy development and project management
