The Diabetes CNS is an advanced nursing practice role that encompasses responsibility and coordination of nursing care to this patient group across health care settings – inpatient, outpatient and community.The CNS works as part of a Diabetes Integrated Team and contributes to the development of integrated service delivery across the continuum of care in conjunction with medical, allied, and non-clinical support roles.
The CNS works closely with the Diabetes Integrated Team to develop evidence-based policies and procedures; and improves the experience for patients and their family/whanau.The CNS works closely with General Practitioner (GP) Services and partner organisations (Tui Ora, Ngāruahine Iwi Authority, Ngāti Ruanui Tahua Ltd and Pinnacle Midlands Health Network).Please click here to view the position descriptionAbout the Team and LocationThe Taranaki Diabetes Integrated Team has two primary functions:1.Comprehensive Clinical & Psychosocial Intervention
- Providing clinical expertise and psychosocial support to directly manage patients with diabetes with the aim of discharging them back to primary care as appropriate.2.
