The Diabetes CNS is an advanced nursing practice role that encompasses responsibility and coordination of nursing care to this patient group across health care settings – inpatient, outpatient and community.The CNS works as part of a Diabetes Integrated Team and contributes to the development of integrated service delivery across the continuum of care in conjunction with medical, allied, and non-clinical support roles.

• Providing clinical expertise and psychosocial support to directly manage patients with diabetes with the aim of discharging them back to primary care as appropriate.2.
